Pleuropulmonary Blastoma in Congenital Cystic Adenomatoid Malformation: Report of a Case

Summary

Pulmonary blastoma is a rare malignant tumor seen in both adults and children. Approximately only 25 % of cases occur in pediatric patients, many of whom affected by a congenital pulmonary cystic lesion. The clinical features, radiological findings and management of a 3-year-old boy affected by a pulmonary blastoma which arose in a congenital cystic adenomatoid malformation are reported, and an extensive review of the literature is also made. Because of the well-known tendency of cystic pulmonary diseases to develop malignancies, authors recommend the surgical excision of these kind of lesion or at least their close radiological follow-up.

Résumé

Le blastome pulmonaire est une tumeur maligne rare vue chez les adultes et les enfants. Approximativement seulement 25 % des cas surviennent chez des patients pédiatriques, beaucoup présentant une malformation kystique adénomatoïde du poumon. L'histoire clinique, l'aspect radiologique et la prise en charge d'un garçon de 3 ans affecté par un blastome pulmonaire, développé à partir d'une malformation congénitale cystique adénomatoïde est rapporté et une revue extensive de la littérature est faite. A cause de la tendance bien connue des léssions kystiques pulmonaires à développer des tumeurs malignes, les auteurs recommandent l'excision chirurgicale de ces lésions ou au moins leur suivi radiologique très précis.

Resumen

El blastoma pulmonar es un tumor maligno raro que se ve tanto en niños como en adultos. Aproximadamente el 25 % de los casos son pediátricos y muchos de ellos están afectados por una lesión quística pulmonar congénita. Presentamos los hallazgos clínicos, radiológicos y el tratamiento de un niño de 3 años afecto de blastoma pulmonar aparecido en una malformación adenomatoide quística congénita así como una extensa revisión de la literatura. A causa de la bien conocida tendencia de las enfermedades quísticas pulmonares a desarrollar procesos malignos, los autores recomiendan la extirpación quirúrgica de estas lesiones o por lo menos su seguimiento muy cercano.

Zusammenfassung

Pleuropulmonale Blastome sind sowohl beim Erwachsenen wie im Kindesalter selten. 27 % der Blastome treten im Kindesalter auf und basieren auf angeborenen, zystischen Lungenveränderungen. In der vorliegenden Arbeit werden die Klinik und die radiologischen Ergebnisse, die Histologie sowie die Behandlung eines pulmonalen Blastoms bei einem 3-jährigen Knaben vorgestellt, das auf der Basis einer zystisch adenomatoiden Fehlbildung entstanden ist. Aufgrund dieser bekannten Tendenz einer malignen Entartung müssen zystische Lungenveränderungen entfernt werden. Langzeitkontrollen sind notwendig.
